The Community Health Access Project works alongside individuals and families to help address the things that most impact everyday life. Support may include help with:

  • Pregnancy, postpartum, and family support

  • Housing stability and utilities

  • Food access and basic needs

  • Transportation to appointments and work

  • Mental and behavioral health connections

  • Employment, education, and financial stability

  • Navigating community resources and benefits

Each person’s situation is different. Our Community Health Workers take the time to understand what’s going on and coordinate support that fits their needs.
