We are happy to share that Shelby Helpline Ministries has joined the Community Health Access Project (CHAP) as our newest Community Care Agency. This partnership strengthens our shared commitment to walking alongside individuals and families who are working toward stability, health, and well-being.

Shelby Helpline Ministries has a long history of caring for their neighbors. Their team is often one of the first places people turn when they need support during a difficult time. By partnering together, we can connect more residents in Shelby and the surrounding area to Community Health Workers (CHWs) who provide one-on-one guidance with things like housing, safe sleep resources, access to medical care, transportation, and more.

What this means in practice is simple: when someone reaches out to Shelby Helpline Ministries with a need, they now have a direct pathway to CHAP’s Pathways Community HUB model. This model focuses on helping individuals move step by step toward their goals while working to reduce the barriers that make life harder. The CHWs will meet people where they are, build relationships, and support progress that lasts.

We are grateful to the leadership and staff at Shelby Helpline Ministries for their willingness to collaborate and their commitment to community health. Partnerships like this help ensure that support is not only available, but accessible, welcoming, and grounded in real relationships.
